Billing Policies
We continually strive to contain costs, while maintaining our commitment to excellence in medical care, by ensuring every appropriate effort is made to collect money owed to the hospital for services provided.
Patients are responsible for the charges for services received. However, to assist patients in meeting their financial obligations, the hospital will bill their health insurance carrier(s) for them, as long as a valid ID card and/or information regarding insurance coverage is presented at the time of registration.
The hospital accepts assignments of benefits and maintains an active follow-up program with all insurance carriers. Insurance is billed as a courtesy to the patient and the patient remains responsible for contacting their insurance carrier to ensure prompt payment of their accounts. Patients should contact their insurance if payment has not been made within 45 days. Accounts with delinquent balances or without adequate payment arrangements may be forwarded to a collection agency or attorney.
At the patient's request, a detailed bill may be provided. The hospital will send periodic statements to the patient or responsible party in an effort to keep them informed as to the status of all open accounts.
Medicare Policies
LVHHN bills Medicare for inpatient and outpatient services. If additional insurance information is provided at the time of service, they will also be billed. Patients are responsible for any charges not covered by Medicare and/or your additional insurance.
Medicare Deductible and Coinsurance Amounts for 2008:
Part A Inpatient Deductible: (pays for inpatient hospital, skilled nursing facility, and some home health care)
For each benefit period Medicare pays all covered costs except the Medicare Part A deductible (2008 = $1,024) during the first 60 days and coinsurance amounts for hospital stays that last beyond 60 days and no more than 150 days. For each benefit period you pay:
- Days 1 – 60: $1,024.00
- Days 61 – 90: $256.00 per day
- Days 91 – 150: $512.00 per day (Lifetime Reserve Days)
- All costs for each day beyond 150 days
Note: You have 60 Lifetime Reserve Days available at day 91
Part B Outpatient Deductible: (covers Medicare eligible physician services, outpatient hospital services, certain home health services, durable medical equipment)
- $135.00 per year. (Note: You pay 20% of the Medicare-approved amount for services after you meet the $135.00 deductible.)
Medicare Deductible and Coinsurance Amounts for 2007:
Part A Inpatient Deductible: (pays for inpatient hospital, skilled nursing facility, and some home health care)
For each benefit period Medicare pays all covered costs except the Medicare Part A deductible (2007 = $992) during the first 60 days and coinsurance amounts for hospital stays that last beyond 60 days and no more than 150 days. For each benefit period you pay:
- Days 1 – 60: $992.00
- Days 61 – 90: $248.00 per day
- Days 91 – 150: $496.00 per day (Lifetime Reserve Days)
- All costs for each day beyond 150 days
Note: You have 60 Lifetime Reserve Days available at day 91
Part B Outpatient Deductible: (covers Medicare eligible physician services, outpatient hospital services, certain home health services, durable medical equipment)
- $131.00 per year. (Note: You pay 20% of the Medicare-approved amount for services after you meet the $131.00 deductible.)
Skilled Nursing Facility Co-insurance
- Days 1 – 20: Medicare Covers
- Days 21 – 100: $124.00 per day
In addition to your deductible, you may be liable for co-insurance.
Blue Cross Policies
We bill all Blue Cross plans for inpatient and outpatient services. Blue Cross plans generally have deductibles and/or coinsurances. The deductible is an annual expense which you must pay before your insurance benefits can begin. Coinsurance is the portion of the total bill which is the patient and guarantor's responsibility to pay.
Commercial Insurance Policies
We bill all carriers for inpatient and outpatient services. Complete billing information required by the carrier or employer must be provided at the time of registration or admission. If your bill is the result of an accident/injury caused by someone else, remember that disputes between you and others do not, and should not, involve the hospital.
